By signing below, I confirm that all information provided is accurate and I am the lawful parent or legal guardian of the participant named above and have the legal authority under UAE law to consent on their behalf. I give permission for the child to participate in football training sessions and events organized by Desert Lions Sports. I understand that the academy is not liable for injuries unless due to negligence.

I give Desert Lions Sports permission to photograph or record my child during training or events for use in official social media, websites, or marketing materials. I understand I may withdraw this consent at any time in writing by contacting info@desertlionssports.com.

I confirm that I have disclosed all relevant medical conditions or allergies and undertake to notify Desert Lions Sports of any future condition that may affect my child’s safe participation. In case of emergency, I authorize Desert Lions Sports to seek appropriate medical assistance to prevent immediate harm. I understand I remain financially responsible for any resulting costs.
