Waiver of Liability Policy

1. Acknowledgment of Risk

The undersigned parent/guardian of the named participant, acknowledge and understand that participation in football training, matches, and related activities organized by Desert Lions Sports involves physical exertion and carries inherent risks, including but not limited to:

Falls, collisions, or contact with other players
Weather-related risks (heatstroke, dehydration, etc.)
Injuries such as sprains, fractures, or concussions
Travel to and from training or match venues

2. Medical Consent

In the event of an emergency, the undersigned parent/guardian authorize the staff of Desert Lions Sports to seek medical treatment for the child. He or She understands that they financially responsible for any medical expenses incurred as a result of injury or treatment.

3. Assumption of Responsibility

The undersigned parent/guardian agree that participation is voluntary and that assumes full responsibility for any injuries, losses, or damages the child may suffer as a result of participation, except where caused by the academy’s gross negligence or willful misconduct.

4. Release of Liability

To the fullest extent permitted by law, the undersigned parent/guardian release and discharge Desert Lions Sports, its directors, coaches, employees, volunteers, and affiliates from any liability, claims, or demands arising out of or connected to participation in academy activities.

5. Media Consent

The undersigned parent/guardian gives permission for the child’s image to be used in academy-approved materials, including social media and printed content. He or She understands that this may revoke this consent in writing at any time.
